Thank you for entrusting the important task of creating the best possible résumé for you to the skilled staff here at Daisy Books.

Right now, you have two choices:
1) If you have a résumé you would like us to work with, you can send it to us as an email attachment (Microsoft Word) or fax it to us. After we receive and review it, we will contact you (usually by email within a few hours of receipt) to flesh out any additional information we may require.

  • if you choose the fax option, please make sure that all of your contact information is both visible and legible.
  • if there is extra information you wish to add, or changes in the information (e.g. A change of address), please include it.

2) You can complete the form below and submit it to us by hitting the "submit" button at the end of it.

Neither choice will affect our speed, skill, and accuracy in the creation of your résumé. So please feel free to choose whichever option is easiest for you.

We already know that you have requested a résumé and paid for it, since notice was transmitted to us by Pay-Pal as soon as you made your payment. Now all we need is the information to create it.

Instructions for Submitting Your Information Using the Form Below:

1) Start with the Name input box. After you type in your name, hit the "tab" button on your keyboard and your cursor will automatically move to the next box. Please note: You will find this function useful since it greatly speeds up the process of completing the form!

2) Please make sure you complete each box.

3) If you want to delete something you’ve typed in, you can do so by backspacing or highlighting the text and hitting your delete button. Or, if you wish to clear the entire form, hit the blue button at the bottom of it (marked "reset"). This will clear the form and allow to you to start over again from the beginning.

4) After you complete the form, hit the red button at the bottom of it (marked "submit form"). This will transmit your form to us.

Describe your short term goals. These guidelines will help you:

  • what kind of position (job) are you seeking?
  • what kind of company or organization do you want to work for?
  • what kinds of responsibilities and experiences do you hope to have in your position?
  • where do you want to live?


    School name, location, degree, coursework etc.


    Job title, company name(s), job description and responsibilities, etc.
    You may also include volunteer experiences in this section.


    List your skills. For example:

  • can you use a word-processor?
  • which computer programs are you competent in?
  • what machinery do you know how to operate? And so forth...


    What are your extracurricular activities, your community activities, hobbies and interests?
    Include any offices you've held and leadership positions, as well as any awards or
    honors you've achieved.
    Please do not abbreviate the names of clubs or organizations you belong to.
